The audiometric health surveillance record for workers exposed above the upper EAV (85 dB(A)). Documents baseline and periodic pure tone audiograms, significant threshold shift detection at 4 kHz, and fitness for work outcomes. NIHL is reportable under RIDDOR. Records must be retained for 40 years.

Covers QSs, estimators, project managers, planners, H&S advisors, and office staff in construction companies. Assesses screen, keyboard, mouse, desk, chair, environment, and work organisation. Self-assessment with manager review satisfies the legal requirement. Extends to home workstations for hybrid workers. Eyesight testing must be provided on request.

HAV / Vibration Risk Assessment and Exposure Record
A risk assessment for hand-arm vibration exposure from construction tools including angle grinders, breakers, compactors, and disc cutters. Documents vibration magnitudes, daily trigger times, calculated A(8) exposure values, and the HSE exposure points system. The EAV of 2.5 m/s² triggers health surveillance; the ELV of 5.0 m/s² must not be exceeded. HAVS is irreversible.

Documents the tiered surveillance system from annual questionnaires through to OH physician assessment, Stockholm Workshop Scale staging, and fitness for work outcomes. Records must be retained for 40 years. HAVS is reportable under RIDDOR when diagnosed.

Noise Risk Assessment and Exposure Record
A risk assessment for noise exposure on construction sites covering common high-noise activities such as angle grinding, disc cutting, percussion drilling, and pneumatic breaking. Documents noise levels, daily LEP,d calculations, and whether the lower EAV (80 dB(A)), upper EAV (85 dB(A)), or ELV (87 dB(A)) are exceeded, with controls following the hierarchy from elimination to hearing protection.

Respiratory Health Surveillance Record
A health surveillance record for workers exposed to respiratory hazards including respirable crystalline silica, wood dust, welding fumes, and isocyanates. Documents baseline and periodic spirometry (FEV1/FVC), respiratory symptom questionnaires, and fitness outcomes. The silica WEL was reduced to 0.05 mg/m³ in 2020. Records must be retained for 40 years.
COSHH Regulations 2002
Whole Body Vibration (WBV) Risk Assessment & Exposure Record
A WBV risk assessment for plant operators exposed to whole body vibration through seats and floors. Covers excavators, dumpers, rollers, compactors, and forklift trucks on rough ground. EAV 0.5 m/s² A(8) triggers controls and health surveillance; ELV 1.15 m/s² A(8) must not be exceeded. Key controls: ground preparation, seat suspension maintenance, speed limits, and job rotation. Distinct from HAV (Q1) which covers hand-transmitted vibration from tools.

Occupational Health Fitness for Work Record
A formal record of occupational health referral outcomes and fitness for work decisions for safety-critical construction roles including working at height, plant operation, and confined space entry. Contains only the fitness outcome (fit / fit with adjustments / unfit) and operational restrictions, not confidential medical details. Must comply with Equality Act 2010 reasonable adjustments and UK GDPR.
