Accident and Incident Report Form
A form for recording details of any accident or incident on site including the injured person, nature of injury, circumstances, and immediate actions taken. It triggers the investigation process and determines whether RIDDOR reporting is required.

Accident Book BI 510
The official accident book (BI 510 format) that must be kept on every construction site with 10 or more employees. It records details of every workplace accident and is a legal requirement under the Social Security Act 1998.

Accident Investigation Report
A structured investigation into the root causes of an accident or incident. It uses methods such as the 5 Whys or fault tree analysis to identify underlying failures and recommend corrective actions to prevent recurrence.

Dangerous Occurrence Report
A report for dangerous occurrences that must be notified to the HSE under RIDDOR 2013 Schedule 2. These include scaffold collapses, crane overturns, explosions, and other specified events regardless of whether anyone was injured.

First Aid Treatment Record
A record of all first aid treatment administered on site. It captures the casualty details, nature of injury or illness, treatment given, and outcome.

First Aider and Equipment Register
A register of qualified first aiders on site including their certification expiry dates, and an inventory of first aid equipment and its locations.

Near Miss Report Form
A form for reporting near-miss events where an incident could have resulted in injury or damage but did not. Near-miss reporting is a leading safety indicator and helps prevent future accidents by identifying hazards before they cause harm.

RIDDOR F2508 Report to HSE
The formal notification to the HSE of a reportable injury, disease, dangerous occurrence, or death under RIDDOR 2013. Fatal and specified injuries must be reported by phone immediately; over-7-day injuries within 15 days.

Occupational Health Referral Form
A form for referring workers to occupational health services for assessment. Common referrals include hand-arm vibration syndrome, noise-induced hearing loss, dermatitis, and musculoskeletal disorders.

Witness Statement Form
A form for collecting witness accounts following an accident or incident. Witness statements should be taken as soon as possible after the event and recorded in the witness own words to preserve an accurate record.
