1. Invisible Hazards Catastrophic Consequences
Excavation work on construction sites exposes workers to hazards that are invisible until the moment they cause harm. Underground services — electricity cables, gas mains, water pipes, telecommunications, and sewers — are buried beneath the surface and cannot be seen. A single strike from a mechanical excavator on a live electricity cable can be instantly fatal. A ruptured gas main can cause an explosion. These are not theoretical risks — cable and pipe strikes cause deaths and serious injuries on UK construction sites every year.
HSG47, the HSE guidance on avoiding danger from underground services, sets out a clear hierarchy for safe excavation: obtain service records from asset owners, carry out a CAT (cable avoidance tool) and genny (signal generator) survey to locate buried services, hand-dig trial holes to confirm the exact position and depth of services, and only then machine excavate — but never within 500mm of a known or suspected service. This hierarchy must be followed in full, in sequence, every time. Skipping any step dramatically increases the risk of a strike.
Excavation collapse is the other primary hazard. Unsupported excavations can collapse without warning, burying workers in seconds. The weight of soil is sufficient to cause fatal crush injuries and suffocation within minutes. If an excavation collapses while a worker is inside, the instinct to jump in and rescue them is understandable but extremely dangerous — secondary collapses are common and frequently kill would-be rescuers. The rule is absolute: NEVER enter a collapsed excavation to attempt a rescue. Call the emergency services immediately.
Underground services are not where drawings say they are
Service drawings show the approximate position of buried services, not the exact position. Services may have been diverted, added, or moved since the drawings were produced. Depths may have changed due to ground movement or previous construction work. The CAT and genny survey locates services in real time, but even this has limitations — it cannot detect all types of service and cannot determine exact depth. Trial holes are the only way to confirm the precise location and depth of a buried service before machine excavation begins.

NEVER enter a collapsed excavation
If an excavation collapses while a worker is inside, do NOT enter the excavation to attempt a rescue. Secondary collapses are common and frequently kill rescuers. Call 999 immediately, keep all persons away from the excavation edges, and wait for the emergency services who have the equipment and training to carry out a safe rescue. This rule is absolute and non-negotiable.

No CAT and genny survey before excavation
Failing to carry out a CAT and genny survey before mechanical excavation is one of the most dangerous mistakes on any construction site. Service drawings alone do not provide sufficient information — they show approximate positions only, and services may have been moved, added, or diverted since the drawings were produced. The CAT and genny survey is a mandatory step in the HSG47 hierarchy and must be completed by a competent person before any ground is broken.
Assuming service drawings are exact
Service drawings from asset owners show the approximate route of buried services. They are not accurate to within centimetres and should never be relied upon as the sole source of information. Services may have been installed at different depths or alignments than shown, and additional services may have been added since the drawings were last updated. Drawings are the starting point — not the final answer. The CAT and genny survey and trial holes are essential to confirm actual positions.
Entering unsupported excavations deeper than 1.2m
Entering an unsupported excavation deeper than 1.2m is extremely dangerous. At this depth, a collapse can bury a worker to chest height or above, making self-rescue impossible and causing fatal crush injuries or suffocation within minutes. All excavations deeper than 1.2m must be supported with shoring, trench boxes, or battered back to a safe angle. A competent person must assess the excavation and confirm it is safe before any worker enters.
No edge protection around excavations
Open excavations without edge protection are a fall hazard for pedestrians, other workers, and vehicles. Barriers, guardrails, or stop blocks must be installed around all excavation edges to prevent falls and to prevent vehicles and plant from approaching too close to the edge. Excavation edges must be kept clear of spoil heaps and heavy loads, which can overload the sides and contribute to collapse.
Not checking for water ingress after rainfall
Water entering an excavation saturates the soil, adds weight to the excavation sides, and dramatically reduces their stability. After any period of rainfall, excavations must be inspected by a competent person before work resumes. Pumps must be available to remove accumulated water. Ignoring water ingress is a direct contributor to excavation collapses — saturated soil can give way without warning.

What is the 500mm rule?▾
The 500mm rule, set out in HSG47, prohibits the use of mechanical excavation equipment within 500mm of a known or suspected underground service. Within this 500mm exclusion zone, all digging must be done by hand using appropriate hand tools (insulated tools where electrical services are involved). This rule exists because mechanical excavators cannot be controlled with sufficient precision to avoid striking a service at close range. The 500mm zone is measured from the outer edge of the service, not from its centre line. If the exact position of the service is not confirmed by a trial hole, the 500mm zone is measured from the position indicated by the CAT and genny survey, with an additional margin of uncertainty.
What should I do if the CAT survey results are inconclusive?▾
If the CAT and genny survey produces inconclusive or ambiguous results, you must not proceed with mechanical excavation in that area. Inconclusive results may indicate the presence of a service that cannot be clearly identified or located. In this situation, hand-dig trial holes must be excavated to investigate the area before any machine excavation takes place. If services are found, their type, depth, and alignment must be recorded and the 500mm exclusion zone applied. If trial holes do not reveal any services and the area is confirmed clear, machine excavation may proceed with caution. Never assume that an unclear CAT reading means there is nothing there.
Who is a competent person for excavation work?▾
Under CDM 2015, a competent person for excavation work is someone with sufficient training, experience, and knowledge to identify the hazards of excavation, assess the risks, and determine the appropriate control measures. For CAT and genny surveys, the operator should hold a current survey competence card. For excavation inspection, the competent person must be able to assess ground conditions, stability, support requirements, water ingress, and proximity to services. On many sites, the competent person for excavation inspection is a site engineer, temporary works coordinator, or a supervisor with specific excavation safety training. The competent person must inspect the excavation at the start of every shift, after any event that could affect stability (rainfall, vibration, nearby loading), and before work resumes after any interruption.
Do the same rules apply to shallow excavations?▾
Yes. The HSG47 hierarchy for avoiding underground services applies to all excavation depths, including shallow excavations. Underground services can be buried at very shallow depths — some cables and pipes are less than 300mm below the surface, particularly in footpaths, verges, and areas that have been previously disturbed. A shallow excavation of 300mm can strike a live electricity cable. The CAT and genny survey, trial holes, and the 500mm exclusion zone apply regardless of the intended depth of excavation. The only exception to the requirement for full excavation support is that very shallow excavations (typically less than 1.2m) may not require trench support, but edge protection and service avoidance measures still apply in full.
