Toolbox Talk — Working at Height

A toolbox talk on the risks of working at height — the single biggest cause of death on UK construction sites.

1. Still the Industry’s Biggest Killer

Falls from height remain the single largest cause of fatal injury in the UK construction industry. In 2023/24, 40 of the 51 workers who died on construction sites were killed by falls from height — approximately 78% of all construction fatalities. This has been the case for decades. Despite significant improvements in equipment, training, and regulation, falls from height continue to kill more construction workers than any other single cause.

The Work at Height Regulations 2005 (WAHR) establish a clear hierarchy of control that must be followed in order. The first duty is to avoid working at height entirely wherever it is reasonably practicable to do so — by designing out the need, by using ground-level assembly, or by carrying out work from below. Where working at height cannot be avoided, the second duty is to prevent falls using collective protection measures such as guardrails, scaffolds, working platforms, and mobile elevating work platforms (MEWPs). Only where collective prevention is not reasonably practicable should the third tier be considered: mitigating the consequences of a fall using personal fall arrest systems such as harnesses, lanyards, and inertia reels.

A harness and lanyard is not the first line of defence — it is the last. A fall arrest system does not prevent a fall from occurring; it arrests the fall after it has happened. The worker still falls, still experiences the force of deceleration, and still hangs suspended until rescue is effected. Suspension trauma — also known as harness-induced pathology or orthostatic intolerance — is a serious and potentially fatal condition that can develop within minutes when a conscious worker is suspended in a harness after a fall. Blood pools in the legs due to the harness straps compressing the femoral veins, and when the worker is eventually rescued and laid flat, the sudden return of pooled, deoxygenated blood to the heart can cause cardiac arrest. Every site using fall arrest systems must have a documented rescue plan that can be executed within minutes of a fall.

The hierarchy is not optional — collective protection before personal protection

The Work at Height Regulations 2005 require the duty holder to follow the hierarchy in order. Jumping straight to a harness without first considering and documenting why guardrails, scaffolds, or MEWPs are not reasonably practicable is a compliance failure. Harnesses are the last resort because they do not prevent the fall, they only arrest it — and the consequences of a fall arrest (impact forces, suspension trauma, rescue delays) can themselves cause serious injury or death.

Check the scaffold tagEvery scaffold on site must display a scaffold tag (usually green for safe to use, red for incomplete or not safe). Before stepping onto any scaffold, check the tag. A green tag confirms that the scaffold has been inspected and is fit for use. A red tag means the scaffold is under construction, being modified, or has been taken out of service. Never use a scaffold with a red tag or no tag at all. If in doubt, do not use the scaffold and report the issue to your supervisor.
Ladders are for access, not a work platformUnder the Work at Height Regulations 2005, a ladder may only be used as a work platform where the risk assessment shows that a more suitable means of access (scaffold, MEWP, podium) is not justified because of the low risk, short duration (no more than 30 minutes in one position), and light nature of the work. Ladders must be Class 1 industrial grade, set at a 75-degree angle (1 out to 4 up), and extend at least 1 metre above the landing point. Three points of contact must be maintained at all times.
Fragile roofs and surfacesFragile surfaces include fibre-cement roof sheets, roof lights, liner panels, glass, and any material that will not support the weight of a person. Falls through fragile roofs are a leading cause of fatal falls in construction. Never walk on or near a fragile surface without appropriate precautions — crawling boards, stagings, or nets beneath. Assume any roof surface is fragile unless it has been confirmed otherwise by a competent person.
Edge protection and openingsAll open edges from which a person could fall 2 metres or more must be protected with guardrails comprising a top rail at a minimum of 950 mm, an intermediate rail, and a toe board. All floor openings, lift shafts, stairwell voids, and any other opening must be covered with a secured, load-bearing cover marked "OPENING — DO NOT REMOVE" or protected with guardrails. Never remove edge protection or opening covers without authorisation and without providing an alternative safeguard.
Falling objectsAny work at height creates a risk of tools, materials, debris, or equipment falling onto people below. Toe boards on scaffolds and working platforms prevent materials from being kicked off the edge. Tool lanyards prevent hand tools from falling. Brick guards and debris netting provide additional protection on scaffolds. Exclusion zones must be established below work-at-height activities to prevent anyone from entering the drop zone. Hard hats are the last line of defence, not the primary control.

Fall arrest is not fall prevention — understand the difference

Fall prevention stops a fall from happening in the first place — guardrails, barriers, scaffolds, and working platforms are fall prevention measures. Fall arrest allows the fall to happen but catches the worker before they hit the ground — harnesses, lanyards, and inertia reels are fall arrest measures. Fall prevention is always preferred because the worker never falls. Fall arrest means the worker does fall, experiences deceleration forces, and must be rescued from suspension. A rescue plan must be in place before any fall arrest system is used, and the plan must be capable of recovering the fallen worker within minutes to prevent suspension trauma.

Using a ladder as a work platform for extended tasks

The Work at Height Regulations 2005 are clear: a ladder should only be used as a work platform where the task is low risk, short duration (no more than 30 minutes in one position), and light work that can be carried out with one hand while maintaining three points of contact. Using a ladder for extended work, for work requiring two hands, or for work involving significant force (drilling, heavy fixing, pulling cables) is a breach of the regulations and a leading cause of falls. A podium step, tower scaffold, or MEWP should be used instead.

Not checking the scaffold tag before use

Scaffold tag systems exist for one reason: to confirm whether a scaffold is safe to use. A green tag means inspected and fit for purpose. A red tag means do not use. No tag means do not use. Despite this simple system, workers routinely step onto scaffolds without checking the tag. Scaffolds under construction or modification may look complete but have missing boards, absent guardrails, or unstable connections. Checking the tag takes three seconds. Falling from an incomplete scaffold takes less than one.

Working near unguarded edges or openings

Open edges and floor openings are predictable fall hazards, and the controls are well established: guardrails for edges, secured covers for openings. Yet workers continue to be fatally injured by falls from unguarded edges and through uncovered openings. The most common failure modes are edge protection being removed and not reinstated, opening covers being moved and not replaced, and workers stepping around or over damaged guardrails rather than reporting them. Every worker on site has a duty to report missing or damaged edge protection immediately.

No exclusion zone established below work at height

Any work at height creates a risk of falling objects — tools, materials, fixings, offcuts, debris. Without an exclusion zone below the work area, people on the ground are exposed to the risk of being struck by falling objects. Exclusion zones must be physically demarcated with barriers and signage, not just verbally communicated. The zone must be large enough to account for objects bouncing or deflecting after impact. Banksmen or marshals may be needed to enforce the zone during high-risk activities such as steelwork erection or roofing.

Harness worn but not connected to an anchor point

A harness that is not connected to a suitable anchor point provides zero protection. This is one of the most frequently observed failures on construction sites — workers wearing harnesses that are not clipped on, or clipped to an anchor point that is not rated for fall arrest loads (minimum 12 kN for a single-user anchor). The harness must be connected to an appropriate anchor point at all times when the worker is exposed to a fall risk. Disconnecting to move between anchor points without using a twin-tail lanyard creates a gap in protection during which a fall would be unprotected.

4. Frequently Asked Questions

Is there a minimum height at which the Work at Height Regulations apply?

No. The Work at Height Regulations 2005 apply to all work at height where there is a risk of a fall liable to cause personal injury, regardless of the height involved. There is no minimum threshold of 2 metres or any other figure. A fall from a stepladder at 1 metre can cause a fractured skull or a broken wrist just as a fall from a scaffold at 10 metres can cause a fatality. The 2-metre figure is sometimes referenced in relation to the requirement for edge protection, but the regulations themselves apply at any height where a fall could cause injury.

Can a ladder be used as a work platform?

A ladder may be used as a work platform only where the risk assessment demonstrates that a more suitable means of access (scaffold, MEWP, podium step) is not justified because the task is low risk, short duration (no more than 30 minutes in one position), and involves light work that can be carried out while maintaining three points of contact at all times. Heavy work, two-handed work, work requiring significant force, and work lasting more than 30 minutes at a single position must not be carried out from a ladder. Where a ladder is used, it must be a Class 1 industrial grade ladder in good condition, set on a firm and level base, at the correct angle (75 degrees, or 1 out for every 4 up), and secured to prevent movement.

What is the difference between fall prevention and fall arrest?

Fall prevention stops the fall from occurring in the first place. Guardrails, scaffolds with full edge protection, working platforms, and MEWPs with guardrails are all fall prevention measures — the worker is physically prevented from reaching the edge or the opening. Fall arrest allows the fall to happen but catches the worker before they reach the ground. Harnesses with lanyards or inertia reels connected to suitable anchor points are fall arrest measures. The Work at Height Regulations 2005 require fall prevention (collective protection) to be used in preference to fall arrest (personal protection) wherever reasonably practicable, because fall prevention eliminates the fall entirely, while fall arrest subjects the worker to fall forces, suspension, and the need for rescue.

What qualification is required to operate a MEWP?

There is no specific legal qualification mandated by name in the Work at Height Regulations 2005, but Regulation 6 requires that any person who works at height must be competent or, if in training, supervised by a competent person. In practice, the construction industry standard is IPAF (International Powered Access Federation) certification for the relevant MEWP category. Category 3a covers scissor lifts (static vertical), category 3b covers boom-type MEWPs (mobile vertical and outreach). Operators must hold a valid IPAF PAL Card for the category of machine being used. Additionally, a familiarisation briefing specific to the make and model of the machine must be completed before first use, even if the operator holds a valid IPAF card.
